- Any Member whose residence is within 30 kilometers from TC FuneCare building and who died on the afore-said place of residence, or within Tagum City, or, died within the 30 kilometers from TC Funecare building and has opted to avail of the TC Funeral Service Packages;
- Any Member whose place of death is beyond 30 kilometers from TC FuneCare building;
- Any Member whose religious beliefs or practices do not allow embalming and/or related funeral service.
- Any Member who died within 30 kilometers from TC FuneCare Building and did not avail of the TC Funeral Services is entitled to only 70% of the mortuary benefit.

Upon arrival at the FuneCare premises, the undertaker shall do the following:
a. Submit initial requirements to the FuneCare Service Associate.
b. The undertaker will transfer the decedent to the embalming table.
c. Label the decedent with the necessary information (Name of the Deceased).
d. Upon arrival of the client/claimant, the FuneCare Service Associate will gather information about the deceased person using the FuneCare Data Sheet.
i. Name of the informant
ii. Name of the deceased person
iii. Location (place where the cadaver is to be picked-up)
iv. Name and contact number of a family member of the deceased person
v. Date and time the information was received
vi. Name of the undertaker (Driver and Helper)
vii. Date and time of death
viii. Age of the Deceased
ix. Cause of Death
x. Date and time of arrival
xi. Service Contract Serial Number
f. The FuneCare Service Associate will facilitate the casket/package selection, once the family decided the funeral package the service contract will be prepared thereafter;
The Following documents shall be signed by the family representative
– Service Contract
– Embalming Consent Form (Before Embalming)
– Refuse to Autopsy (if applicable)
– Data Privacy Act
All data gathered such as name of deceased, wake address, date, time and place of burial, drivers/helper/vehicles assigned and other remarks must be posted in the Interment Board for easy reference of the services rendered
Request to conduct Autopsy

- Proceed to Autopsy
- Restoration Fee
Refuse to conduct Autopsy

- Sign Refuse to Autopsy Form provided by TCF with two(2) witness
- Provide copy to client. Submit to PNP

- (Pauper Casket) 3-5 day waiting days for claimant
- After 5 days without claimant secure blotter to local PNP

- Deliver equipment to the wake location
- Equipment shall be acknowledge by the family representative thru signing the checklist
- Checklist shall be endorse to FuneCare Operations Supervisor
After the Burial
- Assigned driver and helper will retrieve the equipment to wake location
- Return the Equipment to TCF Equipment Storage Area
- Upon Arrival to TCF, the on duty security guard shall check the equipment retrieved thru checklist
- Checklist shall be endorsed to FOS for filing
